In their study, the guided chats followed a script, whereas the unguided chats had no prompts at all. They found that guided chats resulted in deep connections, where peers provided solutions to problems and new perspectives. Their participants referred to unguided chats as pleasant and relaxing acting as a distraction, or temporary relief, from one’s problems. We found that chats with buddies fell somewhere in between what O’Leary et al 24 referred to as guided and unguided chats, which we call a semiguided conversation space. Existing peer programmes that operate in hospitals and community-based settings can be an important resource for patients, caregivers, and health systems. For example, peer supporters offered a calming and supportive presence to a cancer patient during a biopsy that decreased the patient’s anxiety and made the biopsy easier to conduct for the healthcare provider. Programme participants said they learned practical information about managing their health condition through talking with others whom they met through the peer programme. For example, a programme participant in a hospital-based programme who was matched after surgery gained information about how to manage drains after breast cancer surgery to maximize sleep or how to get out of bed when depressed or in pain. She got answers to practical questions such as “how do you take a shower after a surgical procedure? ” (F) A recipient of peer support who participated in a programme for new mothers said, “both peer programs just helped me learn a lot about how to be a good parent.” (C, PR) A peer supporter learned relapse prevention skills and keeping up with her treatments to remain out of the hospital. Peer support programmes also helped people connect to resources for transportation, with navigation of health services, and helped people understand their available options.
It was not only the mental health peer support that was important to participants but also being able to connect over other parts of their lives and identities. The design of Buddy Project provided a sense of anonymity and separation from pre-existing ties, making it easier for participants to disclose struggles; moreover, the pairs appreciated being able to browse each other’s social media pages before connecting. Buddy Project has an explicit mission to prevent suicide and demonstrates this mission across its online platforms, which helps reduce the stigma around mental health within the peer support space. This choice aided the pairs in developing meaningful, compatible, and supportive relationships with each other, where they felt seen and understood. However, the pairs were concerned that matching based on a shared mental health diagnosis may lead to sharing unhealthy coping mechanisms or comparing themselves and the severity of their experiences with their peers.

Task analysis is a user-centered design approach that is implemented to assess whether an individual can complete a task via a technology medium 21. Real-world task analysis included SMS text message exchanges, entering service user goals, completion of surveys by service users, and frequency of contacting the help desk. Peer support specialists were required to send 2 text messages each week to service users and were also instructed to include at least one goal in the smartphone app. The Digital Peer Support Certification was co-produced with peer support specialists and included an education and simulation training session, synchronous and asynchronous support services, and audit and feedback. Participants included 9 certified peer support specialists between the ages of 25 and 54 years (mean 39 years) who were employed as peer support specialists for 1 to 11 years (mean 4.25 years) and had access to a work-funded smartphone device and data plan. A pre-post design was implemented to examine the impact of the Digital Peer Support Certification on peer support specialists’ capacity to use technology over a 3-month timeframe.

Forty-seven of the studies examined physical health and produced 76 effects sizes, and 73 of the studies examined mental health and produced 118 effect sizes (Figure A7 in Online Supplement). Given heterogeneity observed among the results of the studies, we used a random-effects model in conducting meta-analyses and meta-regressions (Higgins et al., 2003; Table 1). For physical and mental health separately, we included a forest plot to visualize the effect sizes and confidence intervals from the included studies, with a computed summary effect size (Figure A8 in Online Supplement). Small effect sizes are below 0.30, moderate effect sizes are between 0.30 and 0.50, and large effect sizes are above 0.50 (Cohen, 1992). First, a systematic search was conducted in December 2023 across PsycINFO, EMBASE and CINAHL because these data bases cover two fields pertinent to this review – social science and medical research. Searches were rerun just before the final analyses in May 2024 to include further articles published in the intervening months.
To create a ToC, it is crucial to engage a variety of stakeholders who can contribute their experiences and insights, making it an iterative process that involves ongoing stakeholder involvement 33. By comprehensively considering how and why an intervention works from beginning to end and within the broader social and environmental context, ToCs help pinpoint problems, benefits, and areas where more knowledge and research are needed. In relation to our second aim involving features of digital peer support interventions, unexpectedly, our synthesis found that greater dosage of digital peer support interventions decreases their effectiveness in bolstering both physical and mental health.
It is important to learn from existing services and platforms that employ technology for mental health peer support in settings outside of controlled research studies (in the real world) to examine users’ perceptions and inform the design of future digital peer support systems. This study aims to uncover potential ways to design digital mental health peer support systems and to outline a set of principles for future designers to consider as they embark on designing these systems. By learning how existing systems are used by people in daily life and by centering their experiences, we can better understand how to design mental health peer support technologies that foreground people’s needs.
We address this gap by taking Buddy Project 25, an online peer support system for mental health and a nonprofit organization, as a case study. Buddy Project is a peer support system interested in fostering friendship, peer support, and connection, which is one aspect of mental health care. Buddy Project is not a replacement for formal treatment or a place to monitor mental health.
Implementation varies between platforms but is either informal (i.e., naturally occurring between untrained peers), formal (i.e., delivered by a peer who has received some training) or a combination of both 11. Peer support has been shown to improve social connectedness, confidence, and knowledge of mental health and reduce feelings of isolation for both the recipient and provider 10,12,13. Furthermore, evidence suggests it develops person-centred skills and has a positive effect on mental well-being whilst being on waitlists 12,14.
By understanding the evolution of peer support, navigating online communities effectively, and integrating digital peer support with other strategies, individuals can harness the power of technology to improve their mental health and well-being. Collectively, these examples underscore the importance of intentional design, active moderation, and accessible communication channels in building peer support networks that effectively contribute to mental health in online learning contexts. Digital peer support interventions are related to improved mental and physical health outcomes across ages, for individuals differing in initial severity of health conditions and across countries. As peer support practice standards are based on experiential learning and sharing of experiences 12, experiential learning was encouraged and an audit and feedback process was incorporated into the second phase of the Digital Peer Support Certification. Audit and feedback is a quality improvement management tool that incorporates a summary of performance over a specific time period designed to provide constructive feedback to people so they can modify their performance 14-16. Audit and feedback is used in all health care settings and most commonly involves clinical health professionals rather than peer support specialists 14-16. For example, if a participant has been seeking help from a therapist, they could suggest their peer to do the same and support their claim with personal experience, making it more likely that their peer seeks additional mental health support. For this to be possible, we argue that individuals would have to be educated on what healthy and unhealthy coping mechanisms are.
